Application Process for Client Services
The Central Valley Regional Center (CVRC) is a regional center that determines eligibility for services for individuals with developmental disabilities. They also coordinate and monitor services, and help families navigate the system of services available for individuals with developmental disabilities. The referral process starts at the regional center because it is the central point for determining eligibility and connecting individuals with the appropriate services.
Step 1: Make a Referral at CVRC.org:
Anyone can refer an individual who is seeking services with Kings Rehabilitation Center, including parents, doctors, teachers, family, friends, or even the individual themselves. Click here to make a referral.
Step 2: Schedule an Interview:
CVRC will schedule an interview within 15 workdays to learn more about the applicant and explain the services involved. The applicant will need to fill out an Intake Assessment Worksheet.
Step 3: Gather Information:
CVRC will gather information about the applicant's medical, psychological, educational, and developmental history. CVRC may also need to do some evaluations.
Step 4: Review and Determine Eligibility:
A team of experts, including a counselor, doctor, and psychologist, will review the applicant's information to determine if they are eligible for services and make recommendations.
Step 5: Explain Results:
CVRC will fully explain the results of the evaluations to the individual and/or their family/guardian.
Step 6: Follow-Up and Referrals:
CVRC will help the applicant find follow-up services and referrals if needed.
Step 7: Assign a Service Coordinator:
If the applicant is eligible for services, they will be assigned a Service Coordinator.
Step 8: Develop a Plan:
The Service Coordinator will help the applicant and their family create a plan for their services. This plan is called an Individual Family Service Plan (IFSP) for children two and under, and an Individual Program Plan (IPP) for those over two.
Step 9: Coordinate Services:
CVRC will help the applicant coordinate the services they need at Kings Rehabilitation Center.
